Staff Medical Registration and Fitness Assessment

As a new member of staff at OOU, completing your medical registration is an important step to ensure you have access to healthcare services throughout your time at the university.

Follow this step-by-step guide to complete your medical registration seamlessly.

All students (undergraduates, postgraduates, and CCED), staff members, and indigenes of the local community need to complete medical registration to access healthcare services provided by the Directorate of Health Services at Olabisi Onabanjo University (OOU).

Medical registration includes a comprehensive medical fitness assessment, which is conducted at the main campus health center.

You will need your letter of appointment, 2 recent passport-sized photographs, and any relevant medical history or health records.

The medical fitness assessment for staff is conducted at the main campus health center.

You can physically visit the main campus health center from the point of obtaining your letter of appointment.

Your medical registration, including the fitness assessment, must be completed at the main campus health center.

Once you have completed your medical registration, you will be officially enrolled in the university’s healthcare system. You will be provided with a clinic card. This grants you access to medical services, consultations, and other healthcare provisions offered by the Directorate of Health Services.

A: Pre-Assessment Stage

1. Account Department (Room 15)

Visit the Account Department at the Directorate of Health Services (DHS).

Make the required payment for the medical fitness assessment.

After payment, you will be issued an official medical receipt.

Ensure the receipt is properly signed and stamped for authenticity.

2. Health Information Management Department (Room 5)

Proceed to Room 5 to submit the following documents:

  1. The official medical receipt from the Account Department.
  2. A photocopy of the medical receipt.
  3. A copy of your employment letter.
  4. One recent passport photograph.

3. Medical Assessment Form (Form B)

After submitting the required documents, you will be given the Health Examination Form B.

This form is necessary for the next medical registration and assessment phase.

Ensure that Form B is properly filled with your details before proceeding to the next steps.

Once you have completed these steps, you are ready to proceed to the next phase, which involves the medical assessment and certification.

B: Assessment and Certification Stage

1. Nursing Department (Room 10)

Take Form B to the Nursing Department.

The nursing staff will conduct initial medical checks, including

  • The measurement of blood pressure.
  • Recording of height and weight.
  • Basic health screening and review of medical history.

After the checks, the nursing staff will record the results on Form B.

2. Medical Laboratory Department (Room 16)

Proceed to the medical laboratory.

Present Form B to the laboratory staff.

Undergo the required laboratory tests, which may include

  • Blood tests (e.g., blood glucose levels, complete blood count).
  • Urine tests.
  • Any other specific tests are indicated on Form B.

The laboratory staff will record the laboratory results directly on Form B.

3. Radiography Department (Room 3)

Visit the Radiography Department for the required chest X-rays.

Present Form B and the radiographer will conduct the necessary imaging tests (e.g., chest X-ray).

4. Consultation Room for Certification (Room 10)

Take the completed Form B, with all test results, to the consultation room.

A medical doctor will review your test results and conduct a final physical examination.

If the doctor finds you fit for work, they will certify your medical fitness on Form B.

The doctor may provide further instructions or follow-up recommendations if any health concerns are identified.

After completing these steps in the assessment and certification phase, Form B will be ready for the final post-assessment steps.

C: Post-Assessment Stage

1. Administrative Office (Room 2)

After the medical doctor certifies your fitness on Form B, take the completed form to the Administrative Office.

The staff will authenticate and stamp your Form B, verifying that all required assessments have been completed.

2. Health Information Management Department (Room 5)

Proceed to the Health Information Management (HIM) Department.

Submit the authenticated and stamped Form B to the HIM staff.

The department will keep a copy of Form B for their records and issue you a copy of the certified medical fitness assessment form for your personal and official use.

3. Clinic/Yellow Card

After submitting Form B, you will be issued your clinic card, a.k.a. the Yellow Card.

This card is important for accessing healthcare services at the DHS.

The clinic card covers both you and eligible dependents (spouse and up to four children under the age of 18).

4. Establishment Building

Take a copy of your certified medical fitness assessment form to the establishment building at OOU.

Submit this copy to the relevant office in the establishment building to complete your staff registration for employment.

Retain any acknowledgement receipt or confirmation that your form has been received.

Once these steps are completed, you will have successfully finished the staff medical registration and fitness assessment process and can proceed with the final stages of your employment formalities.
