Medical clearance is a mandatory step that must be completed at the Main Campus Health Centre, and it involves several stages, including the verification of medical records, a brief consultation with a doctor, and final approval from the administrative office.
Steps to obtaining medical clearance
1. Collect Medical Clearance Form
Obtain the clearance form from your faculty or department office.
This form is crucial as it contains slots for clearance at all necessary directorates of the school, including the Directorate of Health Services (DHS).
Each directorate must sign off to confirm that the student has no pending obligations in their respective areas before the student can proceed with graduation.
2. Gather Necessary Documents
Before heading to the health centre, ensure you have the following documents:
- Clinic Card (Pink card)
- Original Medical Registration Fee Receipt
- 100-level School Fees Receipt
- School Identity Card
3. Main Campus Health Centre
The entire medical clearance process must be completed at the Main Campus Health Centre.
This is the only approved location for the medical clearance procedure for graduating students.
4. Information Management (HIM) Department (Room 5)
Go to Room 5 to verify your medical records.
Here, submit the clearance form and the gathered documents to the HIM staff.
The department will check your records and ensure that all necessary medical data are up to date.
Pay any outstanding medical bills if applicable, and get the HIM staff’s endorsement on your clearance form in the DHS section.
5. Consultation Room (Room 10)
After completing the verification at HIM, proceed to Room 10.
Present your clearance form to the doctor on duty.
The doctor will review your medical history and may conduct a brief consultation.
If all records are satisfactory, the doctor will sign your clearance form in the DHS section, indicating medical approval.
6. Administrative Room (Room 3)
Proceed to Room 3 with the signed clearance form.
The administrative staff will review the form, confirm that all steps have been completed, and apply the official stamp on your clearance form.
This final step completes your medical clearance process for the Directorate of Health Services.
7: Keep a Copy for Your Records
It’s a good practice to keep a copy of the signed clearance form for your records.
This can serve as proof of your clearance in case there are any future discrepancies.
