If you are a registered patient (student, staff, or indigene) at the OOU Health Services, follow these steps to request and obtain your Sick Note Certificate.
1: Visit the Health Centre
Report in person to the university’s health clinic where you received treatment.
You must request the sick note certificate during or after your consultation.
Ensure you have your Pink Card with you, as it serves as proof of registration and helps us access your medical records.
2: Speak with a Medical Officer
After your consultation, request a sick note certificate from the attending medical officer.
The doctor will assess your case and determine if a sick note certificate is necessary based on your medical condition and the treatment received.
3: Processing Time
Sick notes are issued immediately upon request.
In the rare case that you are not able to access them immediately, you should be able to pick them up within 24 to 48 hours after your consultation.
Important Guidelines
- Sick Note Certificates are only issued to patients who have undergone consultations and treatments at our facilities.
- Misuse of sick note certificates, such as falsifying illness or sharing documents with non-registered individuals, is strictly prohibited and will result in disciplinary action.
- Ensure that the certificate is stamped and signed by the medical officer to ensure its validity.
- Sick notes are not to be confused with sick leave certificates, which state the need to stay off work and are issued ONLY at the discretion of the attending medical officer.
