Patients who have received care at any of our facilities can request official medical reports for various purposes, such as academic, professional, or personal needs.
These reports provide a detailed summary of the presenting symptoms, diagnosis, treatment, and recommendations made during care.
Additionally, students and staff who have been treated at external healthcare facilities can submit external medical reports to the DHS.
However, these reports must adhere to specific guidelines before they can be approved by the Directorate and, by extension, accepted by the university.
This process ensures that external medical documents meet the university’s standards for validating medical absences or justifications for missed academic or work-related duties.
Who Can Request a Medical Report?
Only registered patients who have undergone treatment or received care at any of our healthcare facilities are eligible to request a medical report.
The report will contain details about your health condition, medical history, and treatments provided at our facility.
Important Notes
- Confidentiality: Medical reports are confidential and will only be released to the registered patient or, in exceptional cases, an authorised representative with written consent.
- No third-party requests: To maintain confidentiality, medical reports cannot be released to third parties without prior approval.
- Accurate Information: Ensure all the information provided in the request form is correct, as errors can lead to delays in processing your report.
Obtaining a Medical Report
1. Visit the main campus Health Center
To begin the process, visit the Directorate of Health Services, Main Campus.
Make sure to carry your clinic card, as it will be required to verify your identity and access your medical records.
2. Fill Out the Medical Report Request Form
Complete the Medical Report Request Form available at the office.
In some cases, a verbal request for your medical report would suffice.
Be sure to provide accurate details, including your full name, department (if applicable), and registration number.
State the reason(s) for your request as well, (e.g., for academic, employment, or legal purposes).
3. Submit Supporting Documents
In certain cases, you may be required to submit supporting documents.
This can include a letter from your department (for students) or an employer request (for staff).
Ensure you have all the necessary paperwork to avoid delays.
4. Pay the Processing Fee
A processing fee may apply when requesting a medical report.

Payment instructions will be provided by the Health Services Office.
5. Wait for Processing
Medical report processing typically takes between 5 and 10 working days.
You will be informed when your report is ready for collection.
Urgent reports may be processed faster under special circumstances.
6. Collect Your Medical Report
Once your report is ready, you will be notified for pickup.
You must present your clinic card to collect the report.
External Medical Reports for Internal Approval
Students and staff of OOU who need to present external medical reports as proof for medical reasons, such as absence from classwork or official duties, are required to follow specific guidelines for verification and acceptance.
The submitted reports will be carefully reviewed by the Directorate of Health Services (DHS) to assess the legitimacy of the medical claims and approve necessary accommodations or leave.
Important Notice!
- The requesting student or staff member must be duly registered with the Directorate.
- Students who fall sick within the university campus must first report to one of the directorate health facilities closest to them for treatment. If specialised medical care is required, the patient will be referred to the appropriate hospital for care from our facility.
- The student/staff who falls sick outside the university campus and has to attend a private/public hospital must notify the directorate of health services by sending a prior report of his/her state of health condition at the acclaimed health facility within one week of such a situation.
- These patients must immediately after the treatment report to the Directorate of Health Services with a certified and comprehensive medical report.
- The medical report must be typed on the respective hospital letterhead paper with the managing doctor’s name, signature, phone number, and MDCN registration number.
- The reports must be duly stamped with the hospital seal.
- The patient must come with evidence of various financial transactions carried out at the acclaimed health facility within one week of such a situation.
- Every detail of the medical report from the hospital must be correct and factual; any alteration or manipulation attracts sanction. A medical report from traditional healing homes is not acceptable.
- The student/staff are to report to the Main Campus Health Centre for their external medical report review and approval.
Requirements
- A comprehensive medical report.
- The hospital card of the acclaimed health facility.
- The results of the medical laboratory investigations and all other diagnostic investigations carried out.
- Receipts of hospital bill payment
- Prescription sheets, etc.

Having satisfied the above condition, a medical report shall be issued and duly signed by the director of health services.
