Clinic Card

The Clinic Card is an identification card issued by the Directorate of Health Services (DHS) that allows patients, including students, members of staff, and indigenes, to access medical services at any of the OOU health centres.

All registered students, staff members of OOU, and indigenes are eligible; they can receive their cards following completion of their medical registration.

The card includes your name, a unique identification number, and your status as a student, staff member, or non-staff. It also records your visits at the health centre.

The card is essential for accessing medical services at OOU health centers. It ensures that your medical records are properly maintained and helps the healthcare team provide you with timely and efficient care.

Yes, you are required to present your card at each visit to the health center. This helps the staff quickly access your medical records and ensures you receive the appropriate care.

If you lose your clinic card, please report the loss to the Directorate of Health Services, Main Campus, as soon as possible to receive a replacement card.

No, the clinic card is personal and non-transferable. Using someone else’s card is prohibited, considered fraud, and will result in disciplinary action.

If there are changes to your personal information, such as your name or status, please notify us at the Directorate of Health Services, Main Campus, so that your card can be updated.

The clinic card is issued at no cost following completion of your medical registration. However, a replacement fee MAY apply if your card is lost or damaged.

While it’s recommended to always bring your clinic card, in emergency situations, the health centre will provide care. However, you will need to present other means of identification as well as your clinic number (on your card) for your record to be updated when stable.

Yes, the clinic card is valid at all OOU health centres, and you can use it to access services at any of our locations.

The clinic card is a personalised health card that must be presented whenever you visit the university clinic for treatment.

It not only serves as your identification but also contains vital information that can be crucial in medical emergencies.

To ensure that you receive seamless and efficient healthcare, every student, staff member, and indigene is issued a clinic card during medical registration.

Types of Clinic Cards

1. Pink Card (Student Card)

The Pink Card is exclusively for students of Olabisi Onabanjo University.

This card allows students to access all healthcare services provided by the OOU DHS, including general consultations, medical tests, and emergency care.

Each student is issued an individual Pink Card, which is non-transferable and valid only for their healthcare needs at the university.

2. Yellow Card (Staff and Indigene Card)

The Yellow Card is issued to OOU staff members and indigenes of the local community.

This card covers the cardholder and extends healthcare benefits to their dependents.

Dependents include the cardholder’s spouse (if married) and up to four children under 18.

With the Yellow Card, staff and indigenes can access comprehensive healthcare for their families, ensuring that both employees and their loved ones are well taken care of.

Why do you need a clinic card?

The clinic card serves the following important functions:

1. Official Health Identification

The clinic card serves as your official identification when visiting any of our clinics.

It proves that you are registered for health services, ensuring quick and appropriate care without delays.

Without this card, accessing healthcare could become delayed.

2. Contains Vital Health Information

Your clinic card holds key information, such as your health registration number and other personal details.

In the event of an emergency, this data allows clinic staff to quickly access your medical history, background health conditions, and previous treatments, providing life-saving care.

3. Quick Access to Medical Records

Each time you bring your clinic card, our staff can easily retrieve your medical records.

This ensures efficient healthcare delivery, as your health details are readily available, leading to faster and more precise treatment.

4. Prevention of Treatment Delays

Forgetting your Pink Card might lead to delays in treatment or the need to verify your identity and medical history via other means, which could affect the speed of care you receive.

Misuse of the Clinic Card

The Pink Card is issued to each student upon registration and is strictly for personal use. It serves as your unique identifier within the university’s healthcare system and must not, under any circumstances, be shared or transferred to another person. Likewise, the Yellow Card for staff and indigenes is issued to staff members and indigenes for personal use, as well as for their dependents (spouse and up to four children under 18). This card is intended only for the cardholder and their authorised dependents.

Using another person’s Pink or Yellow Card to access healthcare services is a serious violation and constitutes fraud. Any individual caught using someone else’s card or assisting in such fraudulent activities will face disciplinary action. We take the misuse of clinic cards seriously to maintain the integrity of our healthcare system. Please ensure that your card, whether pink or yellow, is used solely by you or your authorised dependents (in the case of the yellow card).

Access Care Without Clinic Card

While your clinic card (pink card) is essential for receiving healthcare at the OOU clinic, we understand that there may be times when you forget or misplace it.

In such cases, here are alternative ways to access healthcare services quickly:

1. Carry a digital photograph of your pink card.

Take a clear photo of the front and back of your Pink Card and store it on your phone.

This can serve as a backup if you forget the physical card.

Ensure that the registration number and other details are visible in the image, as this will help clinic staff verify your identity and retrieve your records.

2. Access Your Student Portal for Proof of Medical Registration

Log in to your OOU student portal using your mobile device.

Most students’ medical registration is linked to their student account.

Once logged in, navigate to the section that displays your health registration status or related documents, and present this to clinic staff as proof of registration.

3. Carry a valid school ID card.

Your OOU student ID card can also help establish your identity at the clinic.

Though it doesn’t replace the Pink Card, it can assist clinic personnel in verifying your registration in the health system.

Report Immediately if the Card is Lost

If your Pink Card is lost or misplaced, find out how to request a replacement as soon as possible.

A replacement card ensures that you always have access to healthcare when needed.
