We provide a range of essential documents to support the healthcare needs of our students, staff, and other university affiliates.

Each of these documents serves a specific purpose, whether it’s facilitating your access to healthcare services, validating your medical status, or fulfilling academic and professional requirements.

1. Clinic Card

The clinic card is an essential document issued to all patients, students, staff, and indigenes upon completion of their medical registration or their first visit to any of our health centres.

It serves as your primary identification within the OOU healthcare system, allowing you to access medical services at our clinics.

2. Sick Leave

This certificate is provided to students and staff who require time off from academic or work responsibilities due to illness.

It officially certifies your need for rest and recovery as recommended by a healthcare professional.

3. Sick Note

This note is a brief document provided to patients as proof of a visit to any of our health facilities for a personal medical complaint.

It serves as an alibi for being away from their scheduled school or work activities.

4. Medical Report

This report is a document that details your medical history and current health status.

It is required for various administrative purposes, including academic deferment, scholarship applications, when transferring medical care to another facility, etc.

5. Medical Clearance

This document serves as proof that the student or staff member has undergone necessary medical assessments and has been cleared of any health-related obligations before departing from the university.

It is an essential part of the exit process for students, retiring staff, or those leaving the university for any reason, ensuring that their health records are properly closed and all medical responsibilities are fulfilled.

6. NYSC Medical Fitness Certificate

This certificate is a mandatory document required by the National Youth Service Corps (NYSC) to verify the physical and medical fitness of graduates before their deployment for national service.

7. Law School Medical Fitness Certificate

This certificate is required by the Nigerian Law School to confirm that prospective law students are medically fit to undertake the rigours of legal education and training.

8. Secondary School Medical Fitness Certificate

This certificate is issued to intending secondary school students, certifying that a student has undergone a health assessment and is physically fit for school activities.

This certificate is often required by secondary schools to confirm the student’s health status before enrolment or participation in physical activities.
