Health Information Management (HIM) Department

The HIM department, also known as the Medical Records Department, plays a crucial role in the overall functioning of the Directorate of Health Services.

Our department is responsible for handling all aspects of patient information, from collecting and storing medical records to ensuring that healthcare data is accurate, secure, and accessible.

We act as the bridge between patient care and the information that supports it, ensuring that medical staff can provide effective and timely care based on accurate patient data.

Our main goal is to collect, review, share, and manage patients’ medical records.

We do this to make sure that important information is easily accessible, helping us provide continuous and high-quality care to patients, plan health services, make informed decisions, conduct quality reviews, and support health research and education.

We take care of all the information generated about a patient during their treatment.

Our commitment to protecting patients’ privacy involves keeping their records secure and ensuring that only authorised people can access them.

Our department follows the guidelines set by the Health Records Officers Registration Board of Nigeria, which was established by Decree 39 in 1989.

Units in the Department

The department is subdivided into sections or units based on the duties performed.

1. Initial Documentation and Registration Unit

Here, we serve as the first point of contact for new students and staff within the health system.

Our job is to ensure accurate documentation of personal and medical information, assigning unique identification numbers to each individual.

We play a vital role in maintaining accurate health records and ensuring that new members of the university community have access to medical services through proper registration.

A. Students

These are newly admitted students into the following university programs:

  1. Pre-Degree Science courses (PDS),
  2. Joint University Preliminary Examination Board (JUPEB),
  3. CCED Diploma,
  4. Regular degree courses,
  5. CCED Degree courses,
  6. Postgraduate courses, etc.
B. Staff

These are the newly appointed staff employed by the university, which can fall under the following categories:

  1. Junior Staff
  2. Senior Staff
  3. Academic Staff
  4. Contract Staff
  5. Staff on sabbatical appointment, etc.

2. Patient Registration Unit

We handle the registration process for all patients seeking medical consultation, including students, staff, and members of the local community.

We make sure that patient details are accurately captured, updating records as needed to maintain a comprehensive and current database.

Our work is crucial in streamlining the consultation process by ensuring that patient information is readily available to healthcare providers.

3. Inpatient Services

In this unit, we manage the flow of information for patients who are admitted for overnight stays at the health centre.

We ensure that all medical records for admitted patients are regularly updated with treatment details, progress notes, and discharge summaries.

Our role is essential for maintaining continuity of care through accurate and timely documentation, which helps in tracking patient management and recovery.

4. Printing and Supply Unit

We are responsible for designing, producing, storing, and distributing various forms and documents used within the Directorate.

This includes medical forms, patient cards, and other essential documentation required for smooth healthcare operations.

We ensure that all forms are available in sufficient quantities and meet the quality standards necessary for effective record-keeping and patient documentation.

5. Statistics Unit

As members of the Statistics Unit, we serve as the custodians of medical records and data, providing insights into patient care trends and outcomes.

We collect, analyse, and compile health data, generating reports that guide decision-making processes and policy formulation within the Directorate.

Our work supports continuous improvement in healthcare delivery by highlighting areas that require attention and tracking the effectiveness of interventions.

6. Medical Records Library

Here, we maintain a well-organised system for storing both active and inactive patient records.

Our active records library houses current patient records for easy retrieval and reference during treatment, while our inactive records library serves as secondary storage for files that are no longer in regular use.

This dual system ensures that all patient records are securely preserved while allowing efficient access when needed for reference or audits.

7. Data Entry/Computer Unit

In the data entry unit, we create and maintain a digital database of medical records.

Our role is to integrate data collection, processing, and reporting, making sure that information is easily accessible to healthcare providers and administrative staff.

By doing this, we enhance the efficiency and accuracy of record-keeping and contribute to improving the quality of care by enabling better data analysis and communication across departments.

8. Administrative Unit

Here, we process various administrative documents for students, staff, and their dependents.

This includes managing the documentation required for accessing healthcare services, such as issuing clinic cards and maintaining eligibility records.

We ensure that all administrative processes run smoothly, supporting the overall functioning of the Health Information Management Department and providing a seamless experience for all users.
