Staff Medical Clearance

For staff members at Olabisi Onabanjo University (OOU) who are retiring, resigning, or leaving the institution for any reason, obtaining a Medical Clearance Certificate from the Directorate of Health Services (DHS) is a vital step in the exit process.

This certificate confirms that the staff member has no outstanding medical obligations and has been cleared to leave the university.

The medical clearance process must be completed at the Main Campus Health Centre, which involves key steps, including the verification of medical records, a consultation with a doctor, and final approval from the administrative office.

Steps to obtaining medical clearance

1. Collect Medical Clearance Form

Obtain the medical clearance form from your department office.

This form is necessary as it contains slots for clearance from various university directorates, including the Directorate of Health Services (DHS).

Each directorate must sign off to confirm that the staff member has no pending obligations before the final exit process can be completed.

2. Gather Necessary Documents

Before visiting the Directorate of Health Services, ensure you have the following documents:

  1. Clinic Card (Yellow card)
  2. Staff Identity Card

3. Visit the Main Campus Health Centre

The medical clearance process must be completed at the Main Campus Health Centre.

This is the only approved location for medical clearance for staff who are retiring, resigning, or leaving the university.

4. Health Information Management (HIM) Department (Room 5)

Go to Room 5, where your medical records will be verified.

Submit the clearance form along with your clinic card and staff identity card to the HIM staff.

The HIM department will verify your medical records to ensure all data is complete and up-to-date.

Settle any outstanding medical bills if there are any, and get the HIM staff’s endorsement on your clearance form in the DHS section.

5. Consultation Room (Room 10)

After completing the verification at HIM, proceed to Room 10.

Present your clearance form to the doctor on duty for review.

The doctor will check your medical history and may conduct a brief consultation to ensure all health matters are resolved.

If everything is in order, the doctor will sign the clearance form, indicating that you have been medically cleared.

6. Administrative Room (Room 3)

Proceed to Room 3 with your signed clearance form.

The administrative staff will review the form, confirm that all required steps have been completed, and apply the official stamp on your clearance form.

This final step completes your medical clearance process for the Directorate of Health Services.

7: Keep a Copy for Your Records

It’s a good practice to keep a copy of the signed clearance form for your records.

This can serve as proof of your clearance in case there are any future discrepancies.
